Pragmatic Free Trial Meta

Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that facilitates research on pragmatic trials. It collects and distributes cleaned trial data, ratings and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological studies to examine the effect of treatment across trials of different levels of pragmatism.

Background

Pragmatic trials are becoming more widely recognized as providing real-world evidence to support clinical decision-making. However, the use of the term "pragmatic" is not consistent and its definition and assessment requires clarification. Pragmatic trials are designed to inform clinical practices and policy decisions rather than verify a physiological hypothesis or clinical hypothesis. A pragmatic study should strive to be as close as it is to actual clinical practices which include the recruiting participants, setting up, delivery and implementation of interventions, determining and analysis results, as well as primary analyses. This is a key distinction from explanation trials (as described by Schwartz and Lellouch1), which are intended to provide a more complete confirmation of a hypothesis.

Methods

In a pragmatic trial it is the intention to inform clinical or policy decisions by demonstrating how the intervention can be incorporated into real-world routine care. This differs from explanation trials that test hypotheses about the cause-effect connection in idealized settings. In this way, pragmatic trials can have a lower internal validity than explanatory studies and be more prone to biases in their design as well as analysis and conduct. Despite their limitations, pragmatic research can provide valuable information for decision-making within the healthcare context.

The PRECIS-2 tool scores an RCT on 9 domains, ranging between 1 and 5 (very pragmatic). In this study, the recruit-ment organization, flexibility in delivery and follow-up domains scored high scores, but the primary outcome and the method of missing data were not at the practical limit. This suggests that a trial could be designed with well-thought-out practical features, yet not damaging the quality.

However, it is difficult to judge how pragmatic a particular trial really is because pragmatism is not a binary attribute; some aspects of a study can be more pragmatic than others. A trial's pragmatism could be affected by changes to the protocol or logistics during the trial. Koppenaal and colleagues found that 36% of the 89 pragmatic studies were placebo-controlled, or conducted prior to licensing. They also found that the majority were single-center. Thus, they are not as common and can only be called pragmatic in the event that their sponsors are supportive of the lack of blinding in these trials.



A common feature of pragmatic studies is that researchers try to make their findings more meaningful by analyzing subgroups within the trial. This can lead to imbalanced analyses and lower statistical power. This increases the possibility of missing or misdetecting differences in the primary outcomes. This was the case in the meta-analysis of pragmatic trials due to the fact that secondary outcomes were not corrected for differences in covariates at the baseline.

Furthermore the pragmatic trials may be a challenge in the gathering and interpretation of safety data. It is because adverse events are typically self-reported and are susceptible to errors, delays or coding errors. The framework was comprised of nine domains that were scored on a scale ranging from 1 to 5 with 1 being more informative and 5 suggesting more pragmatic. The domains included recruitment setting, setting, intervention delivery and follow-up, as well as flexible adherence and primary analysis.

The original PRECIS tool3 featured similar domains and a scale of 1 to 5. Koppenaal and colleagues10 created an adaptation of the assessment, known as the Pragmascope which was more user-friendly to use for systematic reviews. It is crucial to keep in mind that a pragmatic study does not mean a low-quality trial. In fact, there are a growing number of clinical trials that employ the term "pragmatic" either in their abstract or title (as defined by MEDLINE but which is not precise nor sensitive). The use of these terms in titles and abstracts may suggest a greater awareness of the importance of pragmatism but it is unclear whether this is reflected in the contents of the articles.

Conclusions
